Applications are invited from suitably qualified SCHOOL LEAVERS, NON-SCHOOL LEAVERS, and SPECIAL CASES.

UHAS BASIC STUDIES APPLICATION PROCEDURE & ADMISSION REQUIREMENTS 

Entry Requirements: 

• Candidates must possess Ordinary Level, (WAEC) or equivalent with a minimum of five (5) relevant credit passes in subjects which must include English Language for Arts; and English and Mathematics for Sciences and Social Sciences.

Application Procedure

2. Fill the form with the required personal information and attach the relevant documents as stated below.
3. When you are done, kindly send the filled application form to the address within.

Application Checklist:

THIS APPLICATION FORM MUST BE SUBMITTED ALONG WITH THE FOLLOWING DOCUMENTS:

• ENROLLMENT CONTRACT (obtain a copy from the Head of UHAS Basic School)
• MEDICAL FORMS (medical authorization, immunization, health information & examination records)
• COPY OF STUDENT’S BIRTH CERTIFICATE OR PASSPORT BIODATA PAGE
• TWO PASSPORT PHOTOS with a plain background
